The story is based around the relationships  within & between two families who are both struggling with members who are gravely ill. It's set in the US and Shriver uses their problems to highlight, amongst other things, the unreasonable and often barely present health care system in the States. Although this description makes the book sound heavy, it really isn't. Shriver has a wry humour and she writes with acute and often tender observation. This has got to be one of my all time favourite books. Even though I was hugely impressed and affected by 'We've got to talk about Kevin''  this book far surpasses it. I enjoyed it immensely & could not put it down. In my opinion she is one of the best writers around today!
